Ambulance Billing Records Requests
TDFPD does not process Ambulance Billing in-house. If you would like to request a copy of billing records, please contact our billing services:

Medical & Incident or General Records Requests
Any medical, incident or general report requests for official fire department records must be submitted in writing through our application with a copy of your ID and payment. Each request will cost minimum $20.00 and can be paid with either cash, check or credit card (Linked Below).
Your request will not be processed until we have received payment and the appropriate documents.
Forms can be submitted to Amanda Van De Hey, District Specialist avandehey@tahoefire.org or faxed to 775.588.3046.
Per NRS, our staff has until then end of the 5th business day from the request when payment is complete to furnish the public record or respond to the request. The needed documents are attached below.
